1 photo, 4.5 x 3.5 cm, taken within the last 6 months, plain/white background, clear and unedited. Wear clothing that contrasts with the background; avoid white or very light clothing against a white background.

• Use a recent colour photo, in focus, facing the camera with a neutral expression, and without editing or beauty filters.
• Wear clothing that contrasts with the background so the shoulders and body outline remain clear. If the background is white, avoid white or very pale clothing that blends into the background.
• Japan: recent 45 x 35 mm photo, taken within the last 6 months, clear, unedited, with a plain/white background as required for the application.

KTP original/copy as required by the jurisdiction and one copy of the family card. Domicile information is used to determine the submission jurisdiction.

• Prepare a clear, complete copy/scan that is not cropped and keeps all details readable.
• Make sure the name, date of birth, address, and family relationships shown are consistent with the passport and visa form.
• For Japan, domicile information on the KTP/KK is used to determine the application jurisdiction.

Accommodation booking should match the itinerary and stay period. Pasti Visa can assist with preparing hotel booking where needed for the application.

• The booking should show the guest name where possible, accommodation name/address, check-in/check-out dates, and cover the required stay period. Details must be consistent with the itinerary.
• Pasti Visa can assist with preparing hotel booking/accommodation evidence where needed to support the itinerary and Japan visa application.

List activities and accommodation in Japan from entry through departure. Dates must be consistent with flight and accommodation bookings.

• Provide a realistic day-by-day or city-by-city plan: dates, city, main activities, accommodation, and intercity transport. Make sure all dates match the flight and hotel bookings.

Use the account of the person funding the trip. Some jurisdictions may require bank legalization/stamp; if funded by another person, include a guarantee letter and proof of relationship to the guarantor.

• Use an account belonging to the person funding the trip. Make sure the account holder name, account number (if shown), statement period, incoming/outgoing transactions, and balance are clearly readable.

Use company letterhead and verifiable company information. Where employment evidence is required, include at least position, employment start date, and monthly salary; for business travel also state the purpose of visit. It should be recently issued and signed by an authorized company representative.

• For Japan, include the leave/travel period where relevant. For business travel, briefly state the purpose of the visit.
